The UChicago Clinical Pharmacy Internship (CPI) graduated 11 students from the inaugural class July 27th, 2018. Please see below for more information about each intern and their time at UChicago. Congratulations class of 2018!
Nathaniel Johnson
Biography
Nathaniel graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. He is currently completing a Doctor of Pharmacy degree from Roosevelt University College of Pharmacy in Schaumburg, IL. During his summer internship at UChicago, he rounded with the oncology team, collected critical medication histories and counseled patients on their medication therapies. He also collaborated with UChicago clinical and administrative staff to complete metric tracking templates for medication histories, SBAR communication documents for a critical opioid shortage collaboration, phone etiquette protocol to streamline communication between the pharmacy team and other hospital staff and also performed reconfigurations on omnicell systems at various locations throughout the health system. His interests include oncology, operations, internal medicine and informatics. He is looking forward to applying for PGY1 residency and eventual PGY2 training in oncology or pharmacy informatics.
Erin Hermes
Biography
Erin Hermes graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Chicago State University College of Pharmacy. During her summer internship at UChicago she rounded with the Cardiology team and collected critical medication histories for cardiology patients. She is looking forward to applying for PGY1 residency and eventual PGY2 training in cardiology or critical care.
Huda Mirza
Biography
Huda graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Midwestern University Chicago College of Pharmacy. During her summer internship at UChicago she rounded with the Pediatric Team at Comer Children’s Hospital and collected critical medication histories for patients admitted to the Pediatric Intensive Care Unit and the general pediatrics floors. She also collaborated with UChicago clinical and administrative staff to complete an Omnicell optimization project for the omnicells located in Comer Children’s Hospital. She assisted with various drug shortages including the lidocaine drug shortage and assembled kits for the hydromorphone drug shortage. She also had the opportunity to participate in various leadership workshops with the program director, Dr. Bryan McCarthy and had the unique opportunity to shadow him as well.
Ruchi Thakkar
Biography
Ruchi Thakkar graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Midwestern University College of Pharmacy. During her summer internship at UChicago she rounded with the heart transplant team and collected critical medication histories for cardiology patients. She also collaborated with UChicago clinical and administrative staff to complete medication history interviews, discharge counseling, optimized automating dispensing cabinets and assisted with drug shortages. Her interests include cardiology, transplants, and critical care. She is looking forward to applying for PGY1 residency and eventual PGY2 training in transplants or critical care.
Haley Stoller
Biography
Haley graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Midwestern University College of Pharmacy. During her summer internship at UChicago she rounded with the Solid Organ Transplant and Pulmonary team and collected critical medication histories for patients in the ICU. She also collaborated with UChicago clinical and administrative staff to complete an Omnicell reconfiguration project, formulary management, and a medication use evaluation report. Her interests include informatics, academia, and ambulatory care. She is looking forward to applying for PGY1 residency and eventual PGY2 training in internal medicine.
Zahra Siddiqui
Biography
Zahra graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from the University of Illinois at Chicago College of Pharmacy. During her summer internship at UChicago she rounded with the pulmonary critical care team and collected medication histories for newly admitted patients. Additionally, she enjoyed learning antimicrobial stewardship, vancomycin dosing, and clinical interventions from great preceptors. She also collaborated with UChicago clinical and administrative staff to complete a variety of quality improvement projects for the hospital. Her interests include critical care, oncology, and transplant. She is looking forward to applying for PGY1 residency and eventual PGY2 training in critical care or oncology.
Felix Tam
Biography
Felix Tam graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. He is currently completing a Doctor of Pharmacy degree from Roosevelt University College of Pharmacy. During his summer internship at UChicago he rounded with the Liver Transplant team and collected critical medication histories for Liver patients. He also collaborated with UChicago clinical and administrative staff where he completed formulary reviews, counseled on medications, optimized medication cabinets, and created drug dosage interchange reference cards. He is looking forward to applying for PGY1 residency and eventual PGY2 training in transplant or infectious diseases.
Tam Nguyen
Biography
Tam Nguyen graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. He is currently completing a Doctor of Pharmacy degree from University of Illinois College of Pharmacy. During his summer internship at UChicago he rounded with the MICU team and collected medication histories for patients. He also collaborated with UChicago clinical and administrative staff to complete medication formulary review, optimize Omnicell cabinets, and response to drug shortages. His interests include emergency medicine, investigational therapy, and specialty pharmacy. He is looking forward to applying for PGY1 residency and eventual PGY2 training in internal medicine.
Samantha Siepak
Biography
Samantha Siepak graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Midwestern University – Chicago College of Pharmacy. While completing her degree, she also currently works for CVS as a pharmacist intern. During her summer internship at UChicago she rounded with the multidisciplinary team in the Med/Surge unit and collected critical medication histories and built relationships with newly admitted patients. She also collaborated with UChicago clinical and administrative staff to complete drug shortage tasks and assist current residents with multiple projects. Her professional interests include internal medicine, critical care and research. She is looking forward to applying for PGY1 residency, PGY2 training (undecided) and eventual board certification.
Edgar Guzman
Biography
Edgar graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. He is currently completing a Doctor of Pharmacy degree from Midwestern University College of Pharmacy. During his summer internship at UChicago he rounded with the Oncology team and collected critical medication histories for oncology patients. He also collaborated with UChicago clinical and administrative staff to complete medication shortages. His interests include oncology, and cardiology. He is looking forward to applying for PGY1 residency and eventual PGY2 training in Health System Pharmacy Administration.
Kimberly Dang
Biography
Kimberly graduated from the University of Chicago Medicine Clinical Pharmacy Internship program July 27th, 2018. She is currently completing a Doctor of Pharmacy degree from Midwestern University Chicago College of Pharmacy. During her summer internship at UChicago she collected critical medication histories for orthopedic and oncology patients. She also collaborated with UChicago clinical and administrative staff to complete Omnicell reconfiguration, resolve medication shortages, and reconcile medication returns. She is looking forward to applying for PGY1 residency and eventual PGY2 training in oncology or neurology.
